Fall Sports Physical Information pertaining to CDSD High School & Middle School Sports In order to be fully cleared to participate in a fall sport, the following must be done: o Sports Physical and pertinent information can be downloaded from www.cdschools.org, click on “Sports Schedules” and then “Sports Physicals”. o Sports physical paperwork/forms are in the process of being updated and will not be up-to-date on the website until late May. *Will also be made available in all of the MS and HS main offices throughout the summer months. o Sports physical paperwork cannot be dated by an authorized medical examiner (as defined by the PIAA) any earlier then June 1st for the upcoming Fall sports season. o Those attending school offered physicals: o Dates . July 29th at CDHS-8am report time to start screenings. 9am-Physicians arrive. . August 5th at CDEast HS-8am report time to start screenings. 9am-Physicians arrive. o Must have all CDSD and PIAA paperwork completed in its entirety and signed in all applicable areas by the parent/guardian in order to be considered for the physical evaluation or even sports participation. o All Physical paperwork completed by a medical examiner outside of the school offered physicals: Turn all completed paperwork in by the WEDNESDAY (August 11th) PRIOR to the first day of practice to your respective school’s office in order to ensure the student will be on the coach’s list to participate on the first day of practice (provided they are cleared by an authorized medical examiner and any additional paperwork is completed if necessary) as generated by the appropriate personnel of CDSD. o CERTAIN MEDICAL CONDITIONS o Asthma (including a history of Asthma) / Diabetes / Severe Allergic Reaction requiring an epi-pen requires additional paperwork by both the parent/guardian and the physician which must be submitted each school year. . Forms and additional information on this can be downloaded from the sports physical webpage. . Please note that the required script from the physician must state: • Reason for medication • Name of medication • Dosage of medication • How and When dispensed/administered • May carry on their person in school and self administer . Please turn all of this paperwork in with your sports physical to the athletic trainer whom will then forward this to the nurse. . IMPORTANT-If your son/daughter has asthma, regardless of the medication, they must still obtain a rescue inhaler and have the appropriate paperwork on file yearly OR the physician must provide a signed script stating that a rescue inhaler is not needed. . If your son/daughter had one of these conditions on file for a previous sports physical and is considered to no longer have this condition, CDSD requires a script from the physician stating that the student no longer has this condition. • HIGH SCHOOL ATHLETES: CDSD requires all student-athletes participating in all PIAA sanctioned sports at the high school level to take the ImPACT Computerized Baseline Exam. Those th th students entering 9& 11grade, along with those who have not taken the ImPACT exam within the last 2 years must take the exam prior to participating in their sport. This test must be taken at their respective high school only. Testing dates/times and detailed information about ImPACT may be found on the district website within the Sports Physicals link….Testing starts in May for the fall sports season! No student may try-out/participate in these sports until the test has been completed and is up-to-date. YOU MUST SIGN-UP WITH YOUR COACH TO TAKE THIS TEST
